insuranceI am an out-of-network provider and do not bill insurance directly. Upon request, I can provide a detailed statement (often called a “Superbill”) that you may submit to your insurance provider for possible reimbursement, depending on your plan.
I also accept Health Savings Account (HSA) and Flexible Spending Account (FSA) debit cards, which can be used just like a standard credit or debit card.
Requirements for Therapy with SusanClients must:
Live in Texas or plan to travel to Texas for couples therapy.
Desire a couples intensive or online couples therapy.
Be over age 18.
Neither partner poses a threat to oneself or others.

Consumer Access to Healthcare Record and Complaint Information
In accordance with House Bill 4224 and Texas Health and Safety Code Section 181.105, Susan Wade, LCSW provides the following information:
Requesting Access to Healthcare Records
Current and former clients may request copies of records less than 7 years old.
Active clients may request records via the client portal.
Former client records requests must be made via US mail. Requests must be submitted with the Records Request Form and a check or money order in the amount of $50 per record request to cover administrative fees. If you are requesting copies of records of couples therapy, you must submit signed and notarized copies of the Record Request Form from both parties of the couple.

License Violation Reporting
Susan Wade, LCSW is regulated by the Texas Behavioral Health Executive Council. Questions regarding licensure or professional regulation may be submitted through the Council’s website: https://bhec.texas.gov/contact-us/.
Filing a Consumer Complaint
Clients who wish to file a consumer complaint may do so through the Texas Office of the Attorney General Consumer Protection Division. Complaints may be submitted online at: www.TexasAttorneyGeneral.gov/consumer-protection/file-consumer-complaint.
